PacificSource coverage also includes medical management programs and services for those with more intensive medical needs.
- Our Nurse Case Managers work individually with members needing specific help to manage their healthcare. When appropriate, they work with you and your healthcare providers to coordinate your care and achieve optimal quality of life and financial outcomes.
- Our Condition Support Program offers education and support to members with asthma, diabetes, heart failure, chronic obstructive pulmonary disease, coronary artery disease, or juvenile diabetes. Condition Support is available to members insured by large employer groups (those with 51 or more employees), and members on portability policies. Contact us for more information.
- Our AccordantCare rare disease management and specialty pharmacy programs provide individual support and coordination for our members with certain rare diseases, or those requiring injectable medications or biotech drugs.
- Chronic disease self-management: PacificSource partners with local agencies to offer a six-week workshop to help members with chronic conditions self-manage their condition.

As a guardian of a PacificSource Healthy KidsConnect member, you'll appreciate:
- InTouch Health Manager. Powered by WebMD®, Health Manager is an online health and wellness center that provides health education and tools to help you manage your child's health.
- HealthKicks education program. Children nine years of age or who turn nine while covered under the Healthy KidsConnect plan will have the opportunity to enroll in HealthKicks. This fun, educational program is designed to help children develop healthy habits.
- Hospital-based education classes. Reimbursement is available up to $50 per class (up to $150 a year) for eligible health and wellness classes offered by hospitals (some limitations apply; contact us for more information).
- Caremark® Prescription Discount Program. Just show your child's PacificSource ID card at a Caremark network pharmacy and you’ll receive a discount on any prescription drugs not covered by the Healthy KidsConnect plan.
- Prenatal Program. If your teen is expecting, our free Prenatal Program offers support, useful information, and resources during this very important time for her and her baby.
- Tobacco cessation program. Quit for Life® gives your older child the help they need to kick tobacco for good, including one-on-one phone support. This program is available to children 15 and older.
