Community Health Excellence (CHE) Grants

Helping our provider partners deliver innovative care 

Dear Provider Partners:

We recognize that COVID-19 has an unprecedented impact on your practice. PacificSource has taken a number of steps to help support you during this crisis, including launching our Provider Relief Plan and Community Health Excellence (CHE) Responsive Grants program.

Due to an unprecedented number of applications, we have closed the CHE Responsive Grants process.

As with prior years, we will launch our regular CHE grant program that funds healthcare delivery innovations starting in January 2021. Information about the CHE grant program for the 2021-2022 cycle will be available late 2020.

Given the interest we’ve seen for operations support and CHE limitations, we understand that many of our provider partners will still have unmet financial needs related to the COVID-19 crisis. Please be aware that PacificSource is willing to assist providers with financial advances and repayment over time, to help during this unprecedented crisis.
