PacificSource PACE offers comprehensive healthcare and social services for older adults with complex care needs.
PACE is both a healthcare provider and a health insurance plan. We arrange all aspects of your care—freeing you to enjoy your life.
While the PACE model has been growing throughout the country, PacificSource PACE is the first of its kind in Lane County, Oregon. PacificSource, a local, not-for-profit community health plan founded in 1933, is honored to bring this model—and its proven benefits—to our community.

*You may qualify, like most of our participants, for zero-month cost share or premium based on your income. Additionally, all PacificSource PACE services are provided with no co-payments or out-of-pocket expenses to you. Please call us with any questions about your eligibility for zero cost.

The PacificSource PACE Center in Springfield, Oregon
The PACE center serves as a “one-stop shop” where care teams provide a variety of services, including primary care, physical and occupational therapy, social work services, and recreational activities.
Participants can have on-site appointments with highly qualified healthcare professionals, including primary care practitioners, dentists, nurses, physical and occupational therapists and more—then join us for dining, social events, and other life-enriching activities.
For older adults who want to continue living in their community, PacificSource PACE arranges and provides the care they need.
