For claims status unable to be obtained via InTouch for Providers, please complete the Claims Status Request form and email it SECURELY to the emails outlined below, based on line of business. Please note, claims must be at least 90 days past submit date for requests to be completed.
Emailed requests must be separate per line of business and submitted SECURELY using this form to the emails below:
