Often times, patients with Alzheimer's disease or other forms of dementia are supported by a family caregiver—a valuable part of the patient’s care team. Now, these caregivers have a new resource.
PacificSource and Ceresti Health have launched a virtual Caregiver Support Program for family caregivers of Medicare members who have dementia. Ceresti's support program increases caregivers’ knowledge, skills, and confidence by giving them personalized health education and support, access to on-demand resources, a dedicated remote coach, and peer and family engagement.
Caregivers complete assessments on their tablets. These assessments are optimized to detect changes in a patient's condition, and they allow Ceresti to alert providers when health issues emerge that require immediate attention.
