Grand opening marks expanded access to coordinated care for seniors with complex health and social needs
SPRINGFIELD, Ore, July 1, 2026 – Today, PacificSource Health Plans launched PacificSource Program of All-Inclusive Care for the Elderly (PACE) serving eligible older adults in Lane County with complex medical and social needs to help them live safely and independently in their homes and communities.
PacificSource PACE, LLC, is a nationally recognized model that brings medical care, medications, meals, transportation, and social supports together through one coordinated team. The program is designed to reduce fragmentation, improve access to services, and make healthcare easier to navigate for participants and their caregivers.
PacificSource celebrated the opening of its new PACE Center in Springfield, marking the start of services for eligible older adults across most of the Lane County region.
“PACE is designed to close the gaps that make healthcare difficult to navigate,” said Dr. John “Espi” Espinola, CEO of PacificSource Health Plans. “With one interdisciplinary team coordinating care and services, we can reduce barriers and better support participants with complex health and social needs.”
PacificSource PACE offers a wide range of services through a single, integrated care team, including primary and specialty medical care, medications, meals, transportation, life enrichment activities, and support for caregivers and family members.
“Now that our doors are open, our team can provide comprehensive, compassionate care to honor what matters most to participants and their families,” said Jessica Van Fleet‑Green, MD, Chief Medical Officer and Program Director for PacificSource PACE. “When services are coordinated through one interconnected team, participants can spend less time navigating the healthcare system and more time living their lives.”
The launch represents an important milestone for PacificSource as it expands access to coordinated, community-based care for older adults in Lane County. By bringing medical care and supportive services together through one interdisciplinary team, PacificSource PACE helps reduce fragmentation, ease the burden on participants and their families, and support older adults as they live, age, and remain connected in their communities.
