PacificSource D-SNP Update for Lane County and the Portland Metro Area

Beginning in 2026, PacificSource will discontinue its D-SNP plans in Lane County and the Portland metro area. This decision was made with members at the center. D-SNP is designed to provide highly coordinated care between Medicare and Medicaid. Because PacificSource will no longer administer Medicaid in Lane County and the Portland metro area, we would not be able to maintain the level of seamless, integrated care that D-SNP is intended to deliver.  

This change ensures affected members can transition to coverage options that continue to provide coordinated care, while allowing us to deliver stable, integrated care in our remaining D-SNP service areas. 

Our Lane County exit will be on January 31, 2026 to align with our Medicaid exit in Lane County. We are still working with Centers for Medicare & Medicaid Services CMS and Oregon Health Authority (OHA) to determine our timing for our Portland Metro exit. 

Members will receive a notification informing them of the plan closure, the effective date, and where to go for questions or additional support. We are reaching out to affected members directly to help them understand their options and stay connected to the care they need. 

We remain committed to providing accessible care in Oregon communities and will continue serving D-SNP members in our other service areas, including Marion and Polk counties, the Gorge, and Central Oregon. 

What Lane County D-SNP Members Need to Know: 

  • Your PacificSource Medicare coverage will end on January 31, 2026 in Lane County. 
  • Your Medicare and Oregon Health Plan (Medicaid) benefits are not changing. 

  • You need to choose how to get your health and prescription drug coverage.
  • For Lane County D-SNP members, f you do not choose another plan by January 31, 2026, Medicare will select a new prescription drug plan for you, and you will have coverage through Original Medicare starting February 1, 2026.
  • You have a special opportunity to join a new plan any time until March 31, 2026. 

We are still working with CMS and OHA to determine the end date for D-SNP members in the Portland metro area. We will update this page with additional information when we have it. 

Your Options 

Option 1: Join another Medicare health plan 

  • Call 1-800-MEDICARE (1-800-633-4227) or visit Medicare.gov.
  • Medicare health plans cover all Original Medicare services and may include extras like vision, hearing, or dental.
  • Some plans, called D-SNPs, are specifically designed for people with both Medicare and Medicaid. 

Option 2: Switch to Original Medicare 

  • Original Medicare is fee-for-service coverage managed by the Federal Government.
  • If you don’t choose a prescription drug plan by January 31, Medicare will enroll you in a separate prescription drug plan.
  • You’ll receive a notice in November with your new drug plan information. 

Important Timing 

  • Oct 15 – Dec 7, 2025: Medicare Advantage members can switch plans or return to Original Medicare for the following year.
  • Jan 1 – Mar 31, 2026: Medicare Advantage members (except MSA plans) can switch plans or join a standalone drug plan.
  • Medicaid members can make certain changes any month, including:
  • Switching from a Medicare health plan to Original Medicare with a prescription drug plan.
  • Changing prescription drug plans.
  • Enrolling in an integrated D-SNP that covers Medicare and most or all Medicaid benefits.
