PacificSource PACE is for people who:
Participants do not need to be PacificSource members to be eligible.
*Excluding ZIP codes 97498, 97439, 97493, 97390, 97480, 97430, 97324, 97490, and 97461
**As defined by the state of Oregon: Assessed at Service Priority Level (SPL) 1-13 (PDF) as needing help meeting daily health or personal care needs (services not usually covered by traditional health insurance or Medicare)
***In their own home, adult foster home, assisted living facility, or residential care facility
How to get started
Enrollment takes about 30 to 45 days, which can vary depending on Medicaid eligibility, costs, housing situation, medical stability, and other factors.
