What is it?
PacificSource PACE (Program of All-inclusive Care for the Elderly) will provide compassionate, community-based healthcare and social services, with no copays or deductibles, for eligible older adults in Lane County, Oregon.
The PACE model honors what matters most to participants and caregivers. It offers comprehensive, coordinated care in a way that promotes dignity and supports participants wherever they call home.
PacificSource PACE is for people who:
- Are 55 or older
- Qualify for Medicaid or Medicare (or can pay privately)
- Live in Lane County, Oregon (except Zip codes 97498, 97439, 97493, 97390, 97480, 97430, 97324, 97490, and 97461)
- Need long-term services, such as help with daily healthcare or personal care
- Are ready for all their primary care and health services to be coordinated by one team
- Can live safely in the community with help from PACE

Participants do not need to be PacificSource members to be eligible.
What services are included?
- Primary and specialty medical and nursing care
- Prescriptions and over-the-counter medications
- Lab tests and diagnostics
- Medical supplies and equipment
- Dental, hearing, vision, and foot care
- Mental health services
- Therapy and social work services
- Rehabilitation
- Adult day health services, meals, and social activities
- In-home care and services, such as light housekeeping and bathing/showering assistance
- Emergency services
- Transportation
- Other services deemed necessary
Where are services provided?
Our new PacificSource PACE Center in Springfield is the program’s hub. Most healthcare and social services take place here.
Some in-home services are also available, and certain specialist care may be delivered at local practitioners’ offices. We provide transportation to and from all services.
How is care arranged, delivered, and monitored?
Each participant receives healthcare and social services from a team of professionals working together to coordinate their care. This includes primary care, specialty care, physical, occupational, speech therapy, and more. All health services, including primary and specialty care, are received from PacificSource PACE, or an approved provider, unless it is an emergency.
We know being independent and in familiar surroundings is important, but also has its challenges. Our team honors participants’ wishes and what matters most to them, enabling community living for older adults who may otherwise require nursing home care.
Most participants visit the PACE Center to receive any needed care or services, and while at the center they enjoy lunch, activities, and social interaction.
Family members and caregivers can be updated on an ongoing basis with care conferences and personalized plans.
Through the PACE Center and in-home services, we offer comprehensive healthcare and social services to help participants continue to live in the community with dignity and independence.
What are the costs?
Program participants who are eligible for Medicare or Medicaid pay no deductibles, copayments, or coinsurance* for covered services. If you are not eligible for Medicare or Medicaid, you can pay for PACE privately.
PacificSource PACE is both a healthcare provider and a health insurance plan. The PACE model of all-inclusive care is designed to remove financial uncertainty from the lives of participants. This means, for example, that needed prescription drugs and over-the-counter medicines are included as part of the program.
*Depending on insurance eligibility, premiums and/or liability payments may be required and will be reviewed prior to enrollment. Participants may be fully and personally liable for the costs for services that are not approved by PacificSource PACE.
