Coordinating care for you
PacificSource Community Solutions is a local health plan that coordinates care for people on the Oregon Health Plan in specific regions of the state. We work together with local Health Councils of doctors and other healthcare providers to improve the health of communities.
Where we offer the Oregon Health Plan

If you’re a member living in Marion or Polk County on a Kaiser Permanente medical plan, please contact them about your medical benefits at 800-813-2000.
Health Share of Oregon
We also provide medical benefits to some people who live in Clackamas, Multnomah, or Washington Counties through Health Share of Oregon. If you are one of these people, your member ID card will come from Health Share of Oregon and will have "Legacy Health PacificSource” listed under “Physical Health Plan.”
