You may be paid back for part of the mileage costs from your home to your appointment, and back. Contact the ride provider shown at the bottom of this page. They’ll provide a form you’ll need to take with you on your trip. Be sure to submit that completed form within 45 days of the appointment.

As a PacificSource member covered through Oregon Health Plan (OHP), you can get free rides to healthcare appointments. Follow these three easy steps:

Know the address where you'll be picked up and the time and location of your appointment

Tell the person who answers the phone if you have any special needs or additional passengers

Try to plan your ride in advance, but same day transportation may be available, if needed.
